And I would be that the close-in area around Boston actually increased.

Ehhhhh, not necessarily. I've not had the pleasure of trying to call the UA offices in MA but it wouldn't surprise me if it was a 3-hour slog through endless holding, multiple transfers, being disconnected while on hold, and being told different things by different people including the possibility that none were correct.
Sometimes, perhaps even often times, Redditors have sufficiently been there and done that to provide quick, concise info that's not readily or obviously available at the powers that be. *Especially* when dealing with municipalities and public agencies. I mean yes, the info *should* be there and be easy to get. Point is that often isn't the case.

A bit of a hike into Boston but the Athenaeum is amazing.

I've come to appreciate that Boston offers many things. Nightlife however is not one of them.

But it's all sort of wrapped up in the same issue. If demand were there, then presumably sooner or later "the rest" would be legislated/operated to fit.
NYC's transit system runs 24/7/365. Bars are open until 4 and not only is it not especially enforced but many bars and clubs are open well beyond sunrise.

Honestly it’s not the 2am last call. It’s the people. Most bars seem to go dark or close to it by midnight. Sleepy town.

IME this is the rub, especially when it comes to insurance coverage. You can jump through all sorts of hoops to ensure that "the procedure" is covered by your insurance, but then magically the anesthesiologist isn't. At all. And is $25,000 or something ludicrous.
Couple years ago my wife and I held up a fully staffed OR because literally 5 minutes before going in for surgery they put a form in front of us, basically a release, saying that certain aspects *may* not be covered by insurance (including anesthesia/anesthesiologist - they actually called that out), and that we were personally accepting/guaranteeing responsibility for same. This was despite having spent weeks chasing down people at the hospital and our insurance company saying that everything was covered.
So we just refused to sign despite them pressuring us. Magically, 20 minutes later we were cleared for surgery without having to sign the fucking form. Fuck that. Fuck them.

BTW are these prices all-in or are they still subject to "oh that's just for the surgery/surgeon but doesn't include the anesthesiologist" sort of thing?
