The list is of volcanoes, it doesn't specify whether or not they are active or dormant. The most recent discoveries just showed that there are still active volcanoes.

Asphyxiation mainly, combined with being cooked, and crushed or imploded. If it was immediate teleportation, you would pop and sizzle.

Well... I am not sure what sort of technology we have today, which might be able to keep an astronaut alive there. Considering the longest lived lander lasted only about 2 hours.

However, I would strongly advise against instant teleportation to the surface without any protection.
