This study was designed to get this result. It’s complete garbage.

Seems like no control or comparison. If people feel better over time (which is what happens commonly with recurrent AF) you get a positive result. Like doing a study - does eating an apple sure the common cold. Then surveying those eating an apply every day and noticing a trend that most people get better.

But in case that wasn't guaranteed enough of a result: In addition to the topical treatment, all patients were instructed to use laxatives and dietary supplemental fiber.

They added a known treatment to the ointment...

And because the trial didn't have a randomised control. The Dr would have been picking the most promising candidates. The ones who needed surgery or more serious medications (i think they use nitroglycerin for this normally) probable aren't going to be risked on this.

And the ointment was a cocktail of stuff - the hemp may have had nothing to do with it.

Studies like this do only one thing. Allow companies to back up marketing claims.

"In a trial over 90% of people using this product* reduced pain and other symptoms in just 1 week"

*combined with dietary changes, speak to your doctor.

I hope I never have to learn what an anal fissure is.

Silmelinwen t1_j9rvvtr wrote

I hope you never do. I had to have surgery for one after trying to heal it with ointments for 6 months. I was a shell of myself, and could barely care for my baby because the pain was so physically and mentally debilitating. I honestly never thought of how pain could cause someone to want to stop living (and I’ve had major back surgery, gone through child birth, had tonsils out as an adult, had wisdom teeth out w no pain killers after), but that fissure really made me think about how nice it would be for death to take the pain away.

Skele_again t1_j9shirf wrote

Me too. I got one after my daughter was born which (undiagnosed crohns) turned into an abscess. Child birth and wasting away -lost 100lbs w excruciating stomach pain- from crohns over the course of 6 months don't even remotely compare. It's the only time in my life that death seemed like a good option to stop the pain.

My crohns is still active 16 years later & if I get slightly sore down there from all the bathroom trips I PANIC and worry it's a fissure. Legit wonder if I have ptsd.

My wife has a similar story. No kids but got abscesses and fistulas/fissures from untreated Crohns, she lost the same amount of weight as you. It was painful to watch. Do you have anything that has helped? It is likely you have ptsd, it’s common in those with chronic illnesses/autoimmune. I’m hoping you’re feeling better <3. You got this!

Skele_again t1_j9swtiu wrote

Oh I'm sorry to hear she also deals w fistulas! I had one and it was awful! I haven't found much that helps, I got into remission once for a couple years after a bowel resection but otherwise I've not had much luck. I did get to try Entyvio when it first came out but didn't have time to let it work before I needed surgery & there's some new drug out that also has great results in studies that I see a lot of people trying. I hope your wife gets a break from it all and gets a good remission. Take care of yourself too! It's hard to be on the sidelines watching!

Another undiagnosed Crohnie here! I wasn’t diagnosed until after I almost went into sepsis from a perianal abscess and fistula. I was alone in London waiting to meet up with my sister for a planner London/Paris trip. I felt a lump and my dad told me to fly home, but I was stubborn. He gave my sister a bunch of antibiotics to give me (he’s a doctor). I was in pain which turned into excruciating pain and had fever the whole trip. Flew back home to New York and had emergency surgery 36 hours later.

It took 3 surgeries, a seton, and almost 2 years until everything was ok. I’m on remicade now.

grrrrrlar t1_j9uimhy wrote

Wow! Similar to my wife. She’s got two setons now and potentially having another surgery to address her abscess that keeps coming back. She’s on infliximab (inflectra, I think remicade is the same? so many names for one medicine!). She’s almost at the two year mark, and has gotten a lot better- just those pesky abscesses are the main issue!

Seriously, had 5 surgeries over 3 years before mine finally healed. Worst years of my life, literally any moment you can suddenly be in the worst pain of your life.

A little confusing, Does it treat the fissures, or help stop them?

9

Storyteller-Hero t1_j9q8ik9 wrote

Well, if someone has chronic fissures, preventing more would be treatment for the fissures by logic.

tewnewt t1_j9qa715 wrote

That's what I was thinking, but "topical" is generally used after.

nancylyn t1_j9qmiwl wrote

Topical means a cream or ointment that is spread on the skin. Before or after doesn’t matter. You can use a topical medication to prevent a condition or to treat it after it’s occurred.

iwishiwereagiraffe t1_j9r342y wrote

The article says they had 89 test subjects for analysis, and that 80 of them saw improvement to symptoms 1 week after treatment. No claim is made for future prevention, but the findings show further reduction of pain AND reduced instances bleeding after bowel movements over the course of a one month treatment.

So id say treatment.

Prevention may be possible with this method, but it is not explored in the scope of the study.

For those worried about medical logistics and claims: somehow I don't predict the public health data will show a dramatic decline in those seeking prescriptions for lower back pain, corresponding with a dramatic rise in new complaints of chronic assbleeds.
