Personally, I find spooky the situations when I pull a lever in one room and there's a cutscene showing a door somewhere else opening. Especially when there's a time limit to reach then, like in Jak 2/Jak 3 when you have literally 10 seconds of giant rolls and leaps. Or, even weirder, when there's a note on a door saying that I should find another way around 'cause they are locked by some power. You know, those "out of blue" instances when the game is subtly telling you that things are about to get nasty.

Path of Exile: Lunaris Temple level 2. In the background you can hear the tortured screams and the sheer terror. They did such a good job, it makes me so uncomfortable.

when it gets quiet like something about playing a game at night and in the game the soundtrack is taking a pause or some cutscene is going on and it just feels eerie that the surrounding sounds pause for a reason.

Idle animations. Its giving "Player self aware theyve been standing there for 45 minutes"

In Elden Ring, at the Shaded Castle, there is a Revenant hidden under the poisonous water. It comes out only if you approach it and attacks you. If you go on the roof of the nearby building, close enough, stealth, the Revenant will come out and stay there motionless, waiting for you to approach it.

This scared me, knowing what the enemy does, and knowing that I have to go there and fight it, seeing it just sitting there making that heavy breathing noise, waiting. (I attacked it from afar, and it teleported behind me spitting venom, killing me instantly lmao, me being like "oh shit oh shit oh shit").
